Click the Patient Icon

To Add a Patient, click the New Patient button from the Rolodex

NoteNote:  To Edit a Patient's file, double click the patient's name, or highlight the patient and click the Open Patient button.

If you need to Edit the information in the Patient Tab, click the Edit button.

NoteNote: PatientID:  Assigned by Quixote.  (Cannot be changed)

The window below will be empty if this is a new patient. 

Patient Tab Edit

NoteNote:  The Red Boxes correlate with the fields on the HCFA / CMS form.  What you enter in these fields will print on the HCFA.


Name:  Enter the Patient's First and Last Name.  Middle Name is optional. (Box 2 HCFA / CMS)

Nickname:  Enter the Patient's nickname, if applicable.

Spouse:  Enter the Patient's name of Spouse, if applicable.

Account #:  Enter an account #. This is optional.  OwlEx. Some offices file (whoa, did I say file?) by account number.

Status:  Select from the drop down list (This feature helps manage the Patient Rolodex by separating the Inactive and Deceased patients from the Active patients.)

  • Active - All active patients, currently being seen.
  • Inactive - Patients no longer coming in and have zero balances.
  • Deceased - Patients that are deceased.  Should have zero balance.

SSN:  Enter the Patient's Social Security Number

Contact Information:

  • Email:  Enter Patient's Email Address
  • Address: Enter Patient's Address  (Box 5 HCFA / CMS)
  • City: Enter Patient's City (Select from drop down) (Box 5 HCFA / CMS)
  • State: Enter Patient's State (Box 5 HCFA / CMS)
  • ZIP: Enter Patient's ZIP (Box 5 HCFA / CMS)
  • Home Phone: Enter Patient's Home Phone Number
  • Work Phone: Enter Patient's Work Phone Number
  • Ext.: Enter Patient's Work Ext., if applicable
  • Cell: Enter Patient's Cell Phone Number

Demographic Information:

NoteNote:  Green Pencil If you do not see what you are looking for from any drop down window that has a green pencil, click on the green pencil and a Add / Edit window will open for this field.

  • Sex:  Enter Patient's Sex (Select from drop down) (Box 3 HCFA / CMS)
  • Date of Birth: Enter Patient's Date of Birth (Type the Birthdate, or use the drop down calendar) (Box 3 HCFA / CMS)
  • Employed: Enter Patient's Employment Status (Select from drop down) (Box 8 HCFA / CMS)

Exclamation Mark The following 3 items cannot be edited or deleted from the Employed drop down:

  • Employed
  • Full time student
  • Part time student

When Employed is selected: "Employed" will be marked in Box 8 on the CMS / HCFA form

When Full Time Student is selected: "Full-Time Student" will be marked in Box 8 on the CMS / HCFA form

When Part Time Student is selected: "Part-Time Student" will be marked in Box 8 on the CMS / HCFA form

Selecting any of the Employed customized field will result in nothing being marked in Box 8 on the CMS / HCFA form.  Box 8 will be left blank.

  • Occupation:  Enter the Patient's Occupation (Select from the drop down)
  • Employer: Enter the Patient's Employer (Box 11b HCFA / CMS)
  • Marital Status: Enter the Patient's Marital Status (Select from the drop down) (Box 8 HCFA / CMS)

Exclamation Mark The following cannot be edited or deleted from the Marital Status drop down:

  • Single
  • Married

When Single is selected: "Single" will be marked in Box 8 on the CMS / HCFA form

When Married is selected: "Married" will be marked in Box 8 on the CMS / HCFA form

Selecting any of the Marital Status customized field will result in "Other" being marked in Box 8 on the CMS / HCFA form.  

NoteNote:  Employer is not a List (Drop Down option).  Adding a drop down window for Employer would decrease the performance of your software, due to the possible size of this field over time.

How this patient was referred:

Referral Source: The Referral Source is designed to help keep track of your marketing efforts.  It is important to track where your time and money are most effective.  Be very specific.

OwlHere are a few examples:

  • Spinal Screening - Vons Thursday mornings
  • Low Back Workshop - Thursday evenings
  • Yellow Pages - Pacific Bell
  • Yellow Pages - Local Directory
  • Newspaper Ad (which newspaper) November 2008
  • Patient Appreciation Day October 2008
  • Aetna Handbook
  • Existing Patient (Please select Existing Patient and the specific patient from the Rolodex, under patient referral.)

Exclamation Mark Do not enter Referring Physicians here, if you want the Referring Physician printed on the HCFA form,  Box 17 / 17a.  Referring Physicians are entered in the Case Information Tab / Add or Edit Condition.

Printers IconClicking on the Printer Icon will generate a quick Referral Source Report.

Click the Select button to select the referring patient from the Rolodex.

NoteNote: You can filter this report by clicking on the Reports Icon, and the Referral button.  Easily see the top 10 Referring Patients for the week, month, quarter, year, etc.

If you would like to include the User's Photo, click the Photo Tab. Click the center of the add photo window.  Locate the Patient's photo on your hard drive.  Be sure and click the Save button Save button
