Ref. Providers

Referring Providers in this section are Providers whose information must be printed in Box 17 and 17a of the HCFA / CMS form.

Click Admin

Click the Ref. Providers button

Click Add to Add a New Ref. Provider

Highlight Existing Ref. Provider and Click Edit to Edit an Existing Ref. Provider

Highlight Existing Ref. Provider and Click Delete to Delete an Existing Ref. Provider

Complete the infomation under the General Tab.

NOTE:  First Name, Last Name and Title will print in Box 17 of the HCFA / CMS form.

Click the Other Settings Tab

Select the Ref. Provider's Specialty from the drop down list.

Enter the Ref. Provider number(s) that you should print in Box 17a on the HCFA / CMS form.

Be sure to click the Save button.
