Click the Admin Icon

Click the Facilites Button

Enter the Facility demographic information. 

NOTE: The Facility drop down list is only available if Outside Lab Charges are checked in the patient file, case information tab / conditions tab.  The Facility will print in Box 32 of the HCFA / CMS form.
